This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)
Conducts timely, comprehensive assessments of patients’ physical, mental, social, financial, and spiritual needs.
Provides skilled nursing care in accordance with physician/APC plan of care.
Collaborates with patients, caregivers, and multidisciplinary teams to coordinate care.
Evaluates patient response to interventions and adjusts care accordingly.
Develops and implements individualized home care plans.
Ensures compliance with regulatory documentation and standards.
Manages patient caseload and schedules efficiently.
Educates and mentors new nurses, students, and team members.
Participates in agency quality improvement initiatives.
Covers holidays, select weekends, and evening calls on a rotating basis.
Caring practices incorporate compassionate, therapeutic care to promote comfort and healing.
Clinical judgment and action use evidence-based guidelines and critical thinking to manage patient care.
Patient advocacy ensures patients’ concerns and ethical considerations are addressed.
Clinical inquiry engages in continuous learning and quality improvement efforts.
Collaboration works closely with interdisciplinary teams for optimal patient outcomes.
Facilitation of learning educates patients and families about diagnosis, treatment, and self-care.
Response to diversity provides care that respects cultural, spiritual, socioeconomic, and personal differences.
System thinking utilizes healthcare resources efficiently to ensure high-quality patient care.
