Presbyterian Heart Group is seeking a dedicated RN Heart Failure Transition of Care Manager to join our team. In this hybrid inpatient/outpatient role, you will be one of three RNs focused on improving outcomes for patients admitted with acute heart failure.
Your primary responsibility will be to educate patients about heart failure and its management at home, with the goal of reducing morbidity, mortality, and 30-day readmission rates. This role involves both bedside education and follow-up communication with patients post-discharge.
Type of Opportunity: Full Time (1.0 FTE/40 hours per week)
