Presbyterian is seeking a RN Case Manager that will provide clinically-based case management to support the delivery of effective and efficient patient care. The role integrates utilization management, care coordination, and transition planning functions. The Case Manager has the overall accountability for a designated case load and plans effectively in order to meet patients needs, manage the length of stay, and promote efficient utilization of resources. In collaboration with the interdisciplinary team, the case manager supports the physician in facilitating patient care with the objective of enhancing the quality of patient outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers.

Work Schedule: 9hr Days, 4 days a week, rotating saturdays (every third saturday)

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)
Interviews and assesses each patient and family within 48 hours of admission to identify discharge needs, manage expectations, and reduce readmission risk
Applies clinical criteria to monitor appropriateness of admissions and continued stays, referring cases to the Medical Director as needed
Communicates with physicians to ensure documentation supports medical necessity and escalates unresolved cases
Educates patients, families, and interdisciplinary teams on post-acute care options and care coordination services
Develops and evaluates discharge plans with the interdisciplinary team, ensuring timely interventions and continuity of care
Participates in daily multidisciplinary rounds to address care progression and target length of stay
Engages in complex rounds, discharge planning huddles, process improvement teams, and department meetings
Identifies complex psychosocial issues and refers to Social Work, collaborating effectively with Social Work partners
Facilitates patient/family and provider care conferences when needed
Documents assessments, status assignments, interventions, and discharge planning per departmental policies
Ensures safe patient care by adhering to policies, managing time and resources, and maintaining accuracy
